 Telemis : the MC-PACS solution.DR MONDET’s testimonial.

DR MONDET performs approximately 25 studies on a daily basis for the social security in Belgium. As far as medico-legal studies are concerned, he collaborates with more than 120 experts. Unlike most of the official experts, he dedicates more than 80% of his time to these assessments, which equates to approximately 1000 studies annually. In addition he has high-performance IT equipment at his disposal. Diagnostic services, in general, rely on medical studies available on CD-ROM, or on studies performed on site, or subcontracted to an external medical imaging center. This especially relevant for MR and CT scans.

As to the assessments, the study number greatly exceeds, by three to four times, the number realized in a hospital environment. Overall, conventional radiography and echography are combined, and a CT or MR scan is regularly added.

Before installation

DR MONDET : "My challenge is to go filmless by using a CD-ROM for each patient, together with the option to have a paper printout whenever necessary. Among my top priorities is the automatic input of the various CD-ROM formats for my assessments, the archival of pre-selected studies (amongst others, a mammography archive) and to read data from monitors using a user-friendly application".

After installation

1ere Journee Normande de TeleImagerie

La Société Normande d’Imagerie Médicale (SNIM) – délégation régionale de la Société Française de Radiologie (SFR) - le Conseil Régional et l’ARH de Basse Normandie, organisent la 1ère Journée Normande de Téléimagerie le 09 Juin prochain à l’Auditorium du Conseil Régional à Caen.
Cette journée s’inscrit dans le calendrier régional de déploiement de la Télésanté en prévision du lancement de Vikman-Santé. Il s’agit de la première manifestation portant sur un des volets structurants du SROS 3 (télémédecine en imagerie médicale) après sa publication officielle fin Mars 2006.

Telemis s.a. acquires Electrom s.a.s.
 

Ce 21 octobre 2005, la société Telemis a clôturé l’acquisition de la société Electrom s.a.s. en France.  Electrom est fabricant de produits dans l’imagerie médicale par ultrasons.  Cette acquisition renforce la présence directe de Telemis en France et permettra d’y augmenter sa part de marché dans le domaine du PACS (Picture Archiving and Communication Systems) et du traitement d’imagerie médicale en général.

 Filmless at the University Hospitals of Mont Godinne.

On 18th June, the new premises of the Nuclear Medicine department at Mont Godinne were inaugurated.

Dr Krug, deputy head of the department, emphasizes the large amount of new modalities integrated into the PACS. He also brings up the user friendliness of the integrated system now combining the PACS and the Electronic Medical Record Omnipro from the company MIMS : “ The patient’s image history can be retrieved and directly visualized via the TM-ReceptionHE viewer of our PACS; it is even possible to perform a MPR and a MIP (Maximum Intensity Projection) for various studies making it possible to compare PET and CT studies within a few seconds. This facilitates the higher quality of care which directly benefits the patient ”.

Some milestones in the last 4 months were the arrival of two new 16 and 64 slice CT canners, the introduction of CR (Computed Radiology), the start up of the TM-Publisher solution for the burning of CD-Roms for referring physicians, the elimination of traditional X-ray film and the gradual transition from the radiology network to optical fiber in order to cope with the extra workload.

Hubert Meurisse, PACS manager at Mont Godinne shares his comments on the success of a “ filmless ” installation.

From a users point of view :

  • the integration of multiple modalities into the same system positively supports the medical imaging operations realized in an hospital environment;
  • a single but “ secure ” access that offers clinicians a fast and efficient way to make a diagnostic and therapeutic decision;
  • an integration of images in the EPR such that the referring physician obtains a centralized patient view including all clinical and diagnostic data.

From a technology point of view :

  • a single software application presenting data in accordance with specific requirements of each medical imaging modality (MPR for PET, stack mode for CT,
    cineview for XA (coronarography), etc.);
  • a client / server application with remote and secure access, suited to the user and to available system resources;
  • an efficient integration with the EPR both at the server level (e.g. exchange of patient data modifications) and at the client level;
  • a fast and user friendly application designed to replace hardcopy film for all clinicians;
  • a reliable long term storage and archival;
  • a reliable subsitute to hardcopy film for external referring physicians via the TM-Publisher solution.

PACS in the private sector :  the view from the manager of the "Société d'Imagerie" of Bois-Bernard

The importance of a filmless environment for health institutions raises a few important questions:

- How to finance a PACS project in the private sector ?
- How to get an agreement on an investment without direct revues related to it ?

Mr Lejeune, general manager at the S.I.M.B.B. (Pas de Calais, France) puts the emphasis on the financing of PACS by film cost savings. The film consumption for medical imaging and for general radiology of this private clinic counting 231 beds, amounts to 750 KEUR excl. VAT over a 5-year period. The PACS investment (licenses, integration cost, hardware, maintenance) amounts to around 450 KEUR excl. VAT for coverage of 5 years. The objective to save 450 KEUR over a 5-year period is compatible with the theoretical residual cost of radiological film, color prints, CD-Roms and various consumption goods (300 KEUR).

Looking for the most suitable tool in accordance with their way of working, the S.I.M.B.B. chose Telemis-Medical PACS for its flexibilIty, ergonomics and strong evolution potential. From the investor's viewpoint the unlimited number of licenses plays an important role in the success of the deployment of PACS to the institution's clinicians. The implementation of Telemis-Medical started in September 2004. Mr Lejeune adds: "The project phase 1 is operational as from January 2005. Actually, the indicators are being evaluated now and the procedures for this study reconstruction, validated by the radiologists, are compatible with the economic goals : 1 CD + 2 smaller format films 11x14" and a paper print for the 3D reconstructions (for about 1 out of 4 patients).

On top of those film savings, PACS brings about several other not quantifiable savings: savings on waste treatment, savings on printer maintenance and renewal, on the waiting time related to film development, on storage (no need for a dedicated room anymore, no personnel costs related to organization, follow-up , archive, retrieval and distribution).

Finally, many other advantages related to this technology can be cited, such as saving radiologist time, immediate availability of any type of imaging result, the real-time access to the images just as easily via the EPR (Electronic Patient Record) as via the RIS (Radiology Information System), etc. At all levels the patient benefits from a PACS. Moreover, phases 2 and 3, shown in the picture on the first page, should allow the S.I.M.B.B. to deploy PACS by the end of 2005 within the different departments of the institution, e.g. the surgical consultation rooms, the emergency unit, dialysis, the hospitalization unit, etc. Furthermore, it should be possible to connect all medical imaging modalities into an integrated network such as to finally get imaging integrated with the Clinic's EPR and consequently experience a drastic reduction in film expenditure.
